Lembke's core model treats pleasure and pain as opposite sides of a single balance in the brain's reward pathway. A pleasurable act tips the balance toward pleasure and releases dopamine, but the brain, obsessed with homeostasis, immediately tips it an equal and opposite amount toward pain — the comedown or craving. With a single exposure this self-corrects if you wait. But repeated exposure to potent, reinforcing stimuli makes the initial pleasure weaker and shorter while the after-response grows stronger and longer, dragging baseline dopamine below normal into a chronic deficit state experienced as anxiety, irritability, insomnia and depression. The practical upshot: the harder you press the pleasure side, the larger the pain debt you incur, so reward-seeking for its own sake is ultimately self-defeating.
Origin
Developed by Stanford psychiatrist Anna Lembke, chief of the Stanford Addiction Medicine clinic, and set out in her book Dopamine Nation. It synthesizes the neuroscience of the reward pathway from 25 years of clinical addiction practice.

Lembke and Williamson describe someone buying a £200,000 car, feeling briefly thrilled, then fixating on the £250,000 model, then the new driveway lights, then the house upgrade. Each purchase delivers a shorter pleasure and a quicker return to wanting. This is the pleasure-pain balance playing out as consumer behaviour — 'keeping up with the Joneses' is the dopamine system manifesting in the real world.

The pair discuss the old line that 'after copulation the devil's laughter can be heard' — the acute low some people feel after sex. That mild dissatisfaction is evolution's nudge to 'go again', which is exactly why the balance is built this way.
